IF YOU DO NOT AGREE TO THESE TERMS AND CONDITIONS, DO NOT CREATE AN ACCOUNT OR USE THE SERVICE. YOU HEREBY GRANT AGENCY AUTHORITY TO ANY PARTY WHO CLICKS ON THE “BUY NOW” BUTTON OR OTHERWISE INDICATES ACCEPTANCE TO THESE TERMS AND CONDITIONS ON YOUR BEHALF.

DISPUTE RESOLUTION: UNLESS YOU TIMELY OPT-OUT OF ARBITRATION IN ACCORDANCE WITH THESE TERMS AND CONDITIONS, YOU AND WE AGREE THAT ALL DISPUTES BETWEEN YOU AND US OR YOU AND THE MEDICAL GROUPS OR PROVIDERS ARISING OUT OF OR RELATED TO THESE TERMS AND CONDITIONS OR THE SERVICE, WITH LIMITED EXCEPTIONS, WILL BE RESOLVED BY BINDING AND FINAL ARBITRATION AND YOU AND WE WAIVE ALL RIGHTS TO A JURY TRIAL AND TO PARTICIPATE IN A CLASS ACTION LAWSUIT OR CLASS-WIDE ARBITRATION, AS FURTHER SET FORTH BELOW. WE EXPLAIN SOME EXCEPTIONS AND HOW YOU CAN OPT OUT OF ARBITRATION BELOW.

Terms of online appointment

Information collected from this website or provided on any form you have submitted through the website is used only in conjunction with an expressed interest by the user in obtaining additional information about the Care4Life Provider or services. It does not create any type of relationship between the individual providing the information and the Care4Life Healthcare service or any of its Providers. This information is not considered Protected Health Information (PHI) and will be used to contact you because you have requested that you be contacted. In addition, information provided on the website or in any response to you is not and cannot be considered medical advice or treatment. Care4Life Healthcare will not be liable for and you will release and hold them harmless from any and all claims and/or any direct, indirect, consequential, special, exemplary, or other damages of any kind arising there from. Please consult with your doctor for medical advice or treatment for any condition you may have. If you are experiencing a medical emergency, call 911 and/or go to the nearest emergency room.

You agree to use the Services only to seek medical healthcare for yourself or another person. You are solely responsible for all images, information, data, text, messages or other materials (“content”) that you upload, post, publish or display (hereinafter, “upload”) or email or otherwise use via the Service. You will ensure that all content you upload or share complies with all applicable laws, rules and regulations, is appropriate and non-offensive, and that you have all necessary rights to use, share, and/or upload such content, without infringing any third-party rights. Care4Life reserves the right to investigate and take appropriate legal action against anyone who, in Care4Life’s sole discretion, violates this provision, including without limitation, removing the offending content from the Service, suspending or terminating the account of such violators and reporting you to the law enforcement authorities. You agree to not use the Service in a manner that Care4Life determines, in its sole discretion, is illegal or improper.

Fees: Care4Life is cash based and currently working on accepting insurances please check our website at Thecare4life.com for updates. In the event you choose to use the Services to schedule and pay out of pocket for health care services, you will be required to provide Care4Life or Care4Life's financing partner with information regarding your credit card or other payment instrument. You represent and warrant to Care4Life that such information is true and that you are authorized to use the payment instrument. You will promptly update your account information with any changes (for example, a change in your billing address or credit card expiration date) that may occur. You agree to pay Care4Life the amount that is specified in the payment plan in accordance with the terms of such plan and this Terms of Service. If you dispute any charges you must let Care4Life know within sixty (60) days after the date that Care4Life charges you. You shall be responsible for all taxes associated with the Services.

CANCELLATION:  GLP-1's medication Tirzepatide/Semaglutide:   

FEES ARE NON-REFUNDABLE ONCE ORDER IS PLACED. 

YOU SHOULD NOT TAKE  GLP-1's (Tirzepatide/Semaglutide) if you have the following :• If you have a personal or family history of medullary thyroid carcinoma (MTC)  • If you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), • If you are pregnant or breastfeeding,• If you are taking Insulin  •If you have a known allergy to the drug.• It should also be avoided if you have a history of pancreatitis or severe gastrointestinal problems like gastroparesis. 

IMPORTANT: If you have an existing medical condition or disease, you must consult your primary care provider before starting this medication. 

Prescription Services: Care4Life is not a pharmacy and does not hold itself out as a pharmacy. 

CANCELLATION: Telehealth sick visits 

Care4Life’s cancellation policy makes booking, cancelling, and rescheduling easy. Care4Life will honor a full refund for any of our visits when cancelled within the following time constraints:

• Cancel virtual visits up to 3 hours before the scheduled visit

• In-person visits: cancel up to 24 hours of your scheduled appointment time (please check website: thecare4life.com for updates when this service is available.) 

We are unable to offer refunds for the following:

No-shows or missed appointments

Any request made 10 days after the original purchase date

Refund requests due to not receiving a prescription or disagreement with the clinical outcome of the visit

Surgical procedures with specific scheduling requirements

If you do make a same-day appointment by mistake and need to cancel, Care4Life will refund you if you request a cancellation within one hour of completing the purchase.

Florida Use Only: Care4Life Telehealth Sick Visit services are only intended for users that are in State of Florida.

This Service is hosted in the State of Florida and are intended only for users that reside in Florida. Care4Life makes no representation that the materials are appropriate or available for use outside the State of Florida. If you access our Service as a non-Floridian resident, you will be responsible for compliance with all local laws. You agree to comply with all laws and regulations applicable to your use of our Services.

The Service is designed for and intended for Providers, Patients and/or other users that reside in the State of Florida only. We make no representation that the information and services provided on the Service are applicable to, appropriate for, or available to Providers, Patients and/or other users in locations outside the State of Florida. Accessing the Service from territories where the content is illegal is prohibited. If you choose to access the site from a location outside of Florida., you do so on your own initiative, without our consent, and you are responsible for compliance with local laws

Commercial Use: Unless otherwise expressly authorized herein or in the Service, you agree not to display, distribute, license, perform, publish, reproduce, duplicate, copy, create derivative works from, modify, sell, resell, exploit, transfer or upload for any commercial purposes, any portion of the Service, use of the Service, or access to the Service. The Service is for your personal u only.

Prescription Services: Care4Life is not a pharmacy and does not hold itself out as a pharmacy.

Medication is prescribed at the discretion of the Provider and only if medically necessary. Care4Life makes no guarantees that you will get a prescription. If a prescription is required, the Provider will send the prescription electronically to the pharmacy and your medication will be delivered to your address. It is your responsibility to provide the Provider with your correct name, address and phone number. You agree that you are responsible for the cost of all prescriptions sent to your pharmacy. Though our services at Care4life is cash based only, you may use your HSA or FSA to cover the cost of your prescription if applicable. You agree that prescriptions are for your personal use. You also agree to read all the information and labels that accompany your prescription(s) and to contact Care4Life or your healthcare provider with questions. Please note that Providers do not prescribe controlled substances.
